ABSTRACT

Abstract Infectious diseases socially imply individual and community medical problems. Therefore, they require actions aimed at social processes that affect the well-being of the individuals without losing sight of social groups. Faced with this panorama, we ask ourselves: is there a direct relationship between ethics and infectious diseases? To elucidate an answer, let us remember the peak period of the COVID-19 pandemic when guidelines based on ethical principles were issued to facilitate medical decisions on allocating scarce resources in periods of maximum demand. In those moments, since there was no inclusive component of society, the decisions made produced massive criticism. The reactions demonstrated the need to analyze in detail the criteria that had been considered correct. Consequently, we affirm that bioethical principles are transcendental in medical decisions and must be examined, not only for the individual but also with a view to public health. Moreover, the acquired immunodeficiency syndrome (AIDS) epidemic has lived with us for decades, and it continues to show its tragic face in the form of new cases, chronic illnesses, and deaths. Joint United Nations Programme on HIV/AIDS brings us closer to a complex reality where the fight against disease and global health are interrelated with other problems, such as the need to reduce inequality, for which human rights, gender equality, social protection, and the development of research projects, where the ethics committees in research in community processes are constituents.


Resumen Las enfermedades infecciosas implican problemas médicos individuales y comunitarios, por lo que requieren acciones dirigidas a procesos sociales que incidan en el bienestar de los individuos, sin perder de vista a los grupos sociales. Nos preguntamos: ¿existe relación directa entre la ética y las enfermedades infecciosas? Para dilucidar una respuesta, recordemos el periodo más álgido de la pandemia por COVID-19, cuando se emitieron guías fundamentadas en principios éticos para facilitar las decisiones médicas en la asignación de recursos escasos en periodos de máxima demanda. Al no haber un componente inclusivo con la sociedad, las decisiones que se tomaron produjeron críticas masivas, que demostraron la necesidad de analizar a detalle los criterios que se habían considerado correctos. En consecuencia, afirmamos que los principios bioéticos son trascendentales en las decisiones médicas y deben ser examinados, no solo frente al individuo, sino de cara a la salud pública (bien común e individualidad). Por otra parte, la epidemia del SIDA (síndrome de inmunodeficiencia adquirida) convive con nosotros desde hace décadas. ONUSIDA (Programa Conjunto de las Naciones Unidas sobre el VIH/SIDA) nos acerca una realidad compleja, como es que la lucha contra la enfermedad y por la salud global se interrelaciona con otros problemas como la necesidad de reducer la desigualdad, por los derechos humanos, la igualdad de género, la protección social y el desarrollo de proyectos de investigación, donde los comités de Ética en investigación en procesos comunitarios son constituyentes.

ABSTRACT

Abstract Internal marketing is a concept still little recognized and disseminated, with most of the research on this area of marketing focusing on the service sector and well-consolidated and industrialized markets. This article sheds light on the importance of internal marketing practices by identifying their impact on workers' proactive behavior and life satisfaction while observing the effects of affective commitment and job satisfaction. The study collected 428 valid questionnaires from Portuguese workers (218 in the private sector and 210 in the public sector) and applied structural equation modeling to test the hypotheses. The results showed how adopting an internal marketing perspective can contribute to successful organizational and human resource management. It was observed that internal marketing practices lead to affective commitment and job satisfaction in both the private and public sectors, although more pronounced in the private sector. Also, the findings pointed out that the workers' affective commitment leads to adopting proactive behavior and life satisfaction only in the public sector and that job satisfaction leads to proactive behavior and life satisfaction for workers of both the private and public sector. This study contributes to increasing understanding of internal marketing, its applicability, and its importance for workers and organizations. Additionally, comparing the private and public sectors helps understand and show how this practice matches the workers' expectations.


Resumen Este artículo tiene como objetivo arrojar luz sobre la importancia de las prácticas de marketing interno, identificando su impacto en el comportamiento proactivo y en la satisfacción con la vida de los trabajadores, a través de los efectos del compromiso afectivo y la satisfacción laboral. El marketing interno sigue siendo un concepto poco reconocido y difundido. Esta investigación compara estos efectos entre los trabajadores del sector público y privado para reforzar la importancia de dicho concepto. En total, se recogieron 428 cuestionarios válidos de trabajadores portugueses (218 del sector privado y 210 del sector público). Se utilizó el modelo de ecuaciones estructurales para testar las hipótesis de estudio. Esta investigación muestra cómo la adopción de una perspectiva de marketing interno puede contribuir a una gestión organizacional y gestión de personas exitosa. Los resultados mostraron que las prácticas de marketing interno generan compromiso afectivo y satisfacción laboral, tanto en el sector privado como en el público, aunque de forma más acentuada en el sector privado. También se constató que el compromiso afectivo conduce a la adopción de conductas proactivas y a la satisfacción con la vida de los trabajadores del sector público únicamente, y que la satisfacción laboral conduce a la adopción de esta conducta y a la satisfacción con la vida para ambos, sector público y privado. Este trabajo contribuye a una mejor comprensión del concepto de marketing interno y su aplicabilidad e importancia para los trabajadores y las organizaciones. Además, al comparar los sectores público y privado, ayuda a comprender y mostrar cómo esta práctica coincide con las expectativas de los trabajadores de estos dos sectores.


Resumo Este estudo pretende evidenciar a importância das práticas de marketing interno, identificando o seu impacto no comportamento proativo e na satisfação com a vida dos trabalhadores, através dos efeitos do compromisso afetivo e da satisfação no trabalho. O marketing interno continua a ser um conceito com pouco reconhecimento e divulgação, uma vez que a maioria das pesquisas relacionadas com esse campo de marketing é direcionada para o setor de serviços e concentra-se predominantemente em mercados bem estabelecidos e industrializados. Esta pesquisa compara esses efeitos entre trabalhadores do setor privado e público para reforçar a importância deste conceito, sendo que, no total, foram recolhidos 428 questionários válidos junto de trabalhadores portugueses (218 no setor privado e 210 no setor público). O modelo de equações estruturais foi utilizado para testar as hipóteses de investigação. Esta pesquisa mostra como a adoção do marketing interno pode contribuir para o sucesso da gestão organizacional e da gestão de pessoas. Os resultados mostraram que as práticas de marketing interno levam ao comprometimento afetivo e à satisfação no trabalho, tanto no setor privado como no público, embora mais pronunciadas no setor privado. Constatou-se também que o comprometimento afetivo leva à adoção de comportamentos proativos e leva à satisfação com a vida dos trabalhadores, mas apenas do setor público, e que a satisfação no trabalho leva à adoção desse comportamento e à satisfação com a vida para ambos, setor privado e público. Este trabalho contribui para uma melhor compreensão do conceito de marketing interno, da sua aplicabilidade e importância, quer para os trabalhadores, como para as organizações. Além disso, ao comparar os setores privado e público, ajuda a entender e mostrar como essa prática atende às expectativas dos trabalhadores desses dois setores.

Resumen En los últimos años, Colombia registra un aumento tanto de sobrepeso como de obesidad, aspecto que produce el establecimiento de modelos complejos de intervención que generen cambios de actitud, compromiso en las personas y el mejoramiento de hábitos alimenticios y de actividad física. Este estudio pretendió comparar la efectividad entre dos componentes psicológicos (estrategias conductuales tradicionales y un entrenamiento en aceptación y compromiso), sumados al entrenamiento físico concurrente y la educación nutricional (junto con una dieta hipocalórica), sobre variables antropométricas, la aptitud física, los aspectos de la conducta alimentaria y la sintomatología de salud mental. El diseño de investigación fue de caso único ABA (línea base-intervención-línea base) para cada condición de tratamiento. La muestra estuvo conformada por cinco mujeres y un hombre, entre los 33 y 49 años de edad, con preobesidad y obesidad tipo I, asignados entre los grupos de coaching conductual y entrenamiento en aceptación y compromiso. Los hallazgos muestran un mejoramiento en: composición corporal, resistencia aeróbica, fuerza prensil, compromiso y fusión cognitiva frente a situación de alimentación y actividad física en dos participantes de la estrategia de aceptación y compromiso, mientras que hubo mayor variación en los resultados de los participantes de coaching conductual. Este es un pilotaje de un estudio con mayor capacidad de inferencia.


Abstract There is an increase in both overweight and obesity in the world, aspect that tends for the approach of integral models of intervention, that generate changes in attitude, and commitment in people towards their eating habits and physical activity, that counteract a sedentary lifestyle and the influence of counterproductive processed foods for energy balance. This study aimed to compare the effectiveness between two psychological components (traditional behavioral strategies and a 6-session acceptance and commitment training), added together, each one: a) concurrent physical training (strength/endurance) with bioadaptation process, b) hypocaloric diet and c) Nutritional education. Measurements of anthropometric variables were obtained, physical fitness and mental health symptoms, while the average of steps was measured during the whole process, constituting this part in a single ABA case design for each treatment condition. The sample consisted of five women and one man, between 33 and 49 years of age, with pre-obesity and type I obesity, assigned in a non-random way between the behavioral coaching and acceptance and commitment training groups. In physical training, priority was initially given to the conditional capacity of strength, being an incident in the mobilization of large muscle groups (multi-joint) in order to generate a greater caloric expenditure, directly influencing exercises that optimize postural hygiene by working in the core zone and complementing training with synergistic or accompanying biarticular exercises in the movement of daily physical activities, subsequently the execution of aerobic resistance in search of the increase in the maximum oxygen consumption capacity ( max), ending with the implementation of vital flexibility exercises for physical exercise releasing muscle tension, favoring conditional physical ability. Regarding the intervention of the nutritional component, the most important input to begin to identify eating habits and subsequently modify them was the 24-hour reminder, applying in the first week of the baseline and the reversal phase. The nutritional intervention consisted of: (a) education so that participants understood and generated greater adherence to the nutritional proposal and (b) hypocaloric diet, reducing daily intake between 500 and 600 kilocalories per day. A follow-up anthropometric assessment was also carried out towards the third week of the intervention phase to obtain feedback on the incidence of their behavior on the anthropometric variables. Regarding the record of the number of steps, both in the baseline and reversion phases the monitor screens were covered, contrary to the intervention phase, since its observation allows self-monitoring towards the weekly personal goal of steps that was established and accompanied by a professional, who in turn generated a graph with the results of the week and gave feedback to each participant. The findings show an improvement in: body composition, aerobic endurance (VO2 max) and grip strength, being the main factor of result in the realization of the physical exercise. Likewise, an increase in the number of steps from the baseline is identified in both groups (X = 9 228.07; S = 1 985.89) to the intervention phase (X = 9 734.22; S = 1 660.86) and a decrease in reversion (X = 9 099.88; S = 2 328.69). In the acceptance and commitment group, no increase in mental health symptoms was identified, increased or sustained cognitive restriction and decreased scores for uncontrolled eating and emotional eating, while there was greater variation in the results of the behavioral coaching participants. This is a pilot of a study with greater capacity for inference.

Resumen En esta investigación de corte psicométrico se analizó la validez y la confiabilidad de la Escala Triangular de Amor de Sternberg (ETAS) de 45 ítems, en una muestra de 296 personas de la ciudad de Arequipa (ubicada al sur del Perú). El 56.1% fueron mujeres y el 43.9% fueron varones con una edad promedio de 40.7 años. La ETAS tiene tres componentes: Intimidad, Compromiso y Pasión. Para el procesamiento estadístico se aplicó el análisis factorial confirmatorio para analizar la estructura interna y la prueba alfa ordinal para estimar la confiabilidad. Se encontró que la ETAS tiene una estructura de tres factores, luego de eliminar 11 ítems debido a que saturaban en dos o más factores. La escala quedó reducida a 34 ítems, con adecuados índices de bondad de ajuste y una confiabilidad con índices superiores a .93 en cada uno de sus tres factores.


Abstract In this psychometric research, the validity and reliability of the 45-item Sternberg Triangular Love Scale (STLS) was analyzed in a sample of 296 people from the city of Arequipa (located in southern Peru). 56.1% were women and 43.9% were men with an average age of 40.7 years. The STLS has three components: Intimacy, Commitment and Passion. For statistical processing, confirmatory factor analysis was applied to analyze the internal structure and the ordinal alpha test to estimate reliability. It was found that the STLS has a three-factor structure, after eliminating 11 items because they loaded on two or more factors. The scale was reduced to 34 items, with adequate goodness-of-fit indexes and reliability with indexes greater than .93 in each of its three factors.

ABSTRACT

Objective:To investigate the effect of online acceptance and commitment therapy(ACT) group intervention on psychological flexibility of undergraduate students with facial negative physical self.Methods:From March to June 2022, thirty-two undergraduates with facial negative physical self-image were recruited, assessed and selected from a university in Zhejiang Province.The subjects were randomly divided into the training group( n=16) and the control group( n=16). The training group received 20 days of online ACT group intervention, while the control group kept waiting.The cognitive fusion questionnaire(CFQ), acceptance and action questionnaire version Ⅱ(AAQ-Ⅱ) and negative physical self scale-face(NPSS-F) were used to measure the two groups before and after intervention and 1 month after intervention.Repeated measurement analysis of variance(ANOVA) conducted was used to compare scale results with SPSS 26.0. Results:(1)The interaction effect between group and time of CFQ scores was statistically significant( F(1.625, 48.755)=4.316, P=0.025, ηp2=0.126). After intervention, the CFQ score of the training group was significantly lower than those of the control group(34.38±8.66, 40.94±8.82, P<0.05). The CFQ scores of the training group after intervention and 1 month after intervention(36.00±9.49) were both significantly lower than those before intervention(41.87±8.72, all P<0.05). (2)The interaction effect between group and time of AAQ-Ⅱ scores was marginally statistically significant( F(2, 29)=3.009, P=0.065, ηp2=0.172). After intervention, the AAQ-Ⅱ score of the training group was significantly lower than those of the control group(24.12±4.97, 28.69±6.78, P<0.05). The AAQ-Ⅱ score of the training group after intervention was significantly lower than those before intervention(28.75±6.11, P<0.05). (3)The interaction effect between group and time of NPSS-F scores was not statistically significant( F(2, 29)=1.509, P=0.238, ηp2=0.094), while the main effect of time was marginally statistically significant between the two groups( F(2, 29)=3.315, P=0.051, ηp2=0.186). The NPSS-F score of the training group after intervention was significantly lower than those before intervention(16.06±6.67, 18.81±7.85, P<0.05). Conclusion:Online ACT group intervention can effectively reduce the cognitive fusion and experiential avoidance of undergraduate students with facial negative physical self, then can improve their psychological flexibility.

ABSTRACT

Objective:To explorethe effect of acceptance and commitment therapy on self-disclosure, coping style and post-traumatic growth in patients with ovarian cancer undergoing chemotherapy after surgery.Methods:A total of 84 patients with ovarian cancer undergoing chemotherapy after surgery were recruited from the gynecology ward of the First Affiliated Hospital of University of Science and Technology of China for randomized controlled trial, from February 2022 to October 2022. All participants were divided into the intervention group and the control group with 42 patients in each group by random number table method. The patients in control group received routine care. The intervention group was given acceptance and commitment therapy (ACT) on the basis of the control group and intervened for three cycles of chemotherapy. The scores of Distress Disclosure Index (DDI), Cancer Coping Modes Questionnaire (CCMQ), and Post-Traumatic Growth Inventory (PTGI) were compared between the two groups before and after intervention.Results:There was no significant difference in the scores of DDI, CCMQ and PTGI between the two groups before intervention ( P>0.05). After intervention, DDI scorein intervention group was (38.81 ± 5.96) points, significantly higher than that in control group (34.43 ± 4.79) points, the difference was statistically significant ( t = 3.71, P<0.01). In terms of coping styles, after intervention, the scores of five dimensions of fantasy, resignation, avoidance, catharsis and confrontation were 6.00(6.00, 8.00), 9.00(8.00, 12.00), 9.00(8.75, 11.00), 7.00(6.00, 8.00) and 20.00(16.00, 21.00) points in the invention group, compared with the control group of 8.00(7.75, 9.00), 11.00(9.75, 13.00), 11.00(9.00, 13.00), 9.00(8.00, 12.00) and 16.00(13.00, 18.50) points, the differences were statistically significant ( Z = 2.86 to 5.11, all P<0.01). The total PTGI score in intervention group was (71.43 ± 8.68) points, significantly higher than that in control group(63.98 ± 6.92) points, the difference was statistically significant ( t = 4.35, P<0.01). Conclusions:ACT can increase self-disclosure, enhance positive coping, and promote post-traumatic growth in ovarian cancer patientsundergoing chemotherapy after surgery.

ABSTRACT

Objective:To explore the intervention effect of acceptance and commitment therapy on the psychological flexibility, self-compassion, anxiety and depression of the primary caregivers of patients with primary liver cancer, so as to provide reference for the clinical care of the primary caregivers of cancer patients.Methods:This was a prospective study. A total of 80 primary caregivers of patients with primary liver cancer treated in Tianyou Hospital Affiliated to Wuhan University of Science and Technology from July to December 2021 were selected as the observation objects, and they were randomly divided into the control group and the intervention group according to the random drawing method, with 40 cases in each group. The control group was given routine nursing measures, while the intervention group was given acceptance and commitment therapy on the basis of routine nursing. The intervention effect was evaluated by the Acceptance and Action Questionnaire-2nd Edition (AAQ-Ⅱ), Self-Compassion Scale Short-Form (SCS-SF) and Hospital Anxiety and Depression Scale (HADS) before the intervention, on the day of discharge, and one month after discharge.Results:Finally, 71 primary caregivers completed the intervention and follow-up, 36 in the control group and 35 in the intervention group. Before the intervention, there was no statistically significant difference in the scores of AAQ-Ⅱ, SCS-SF, Anxiety subscale of HADS(HADS-A) and Depression subscale of HADS(HADS-D) between the two groups ( P>0.05). The AAQ-Ⅱscores of the intervention group on the day of discharge and one month after discharge were (19.63±2.59), (19.12 ± 2.20) points, which were significantly lower than those of the control group (23.14 ± 2.49), (22.56 ± 2.40) points. The differences were statistically significant ( t=5.83, 6.25, both P<0.01). The SCS-SF scores of the intervention group on the day of discharge and one month after discharge were (39.34 ± 2.68), (39.89 ± 2.81) points, which were significantly higher than those of the control group (36.69 ± 3.08), (37.72 ± 2.41) points, the differences were statistically significant ( t=-3.86, -3.49, both P<0.01). The HADS-A/HADS-D scores of the intervention group on the day of discharge and one month after discharge were (9.31 ± 1.95), (9.09 ± 1.60) points and (8.80 ± 2.15), (8.54 ± 1.75) points,which were significantly lower than those of the control group(11.42 ± 1.50), (11.03 ± 1.70) points and (10.11 ± 1.92), (10.03 ± 1.84) points, the differences were statistically significant( t values were 2.71-5.10, all P<0.01). The scores of AAQ-Ⅱ, SCS-SF, HADS-A and HADS-D of the two groups were analyzed by repeated measures analysis of variance, and there were significant differences in time effect, inter-group effect and interaction effect ( F vaules were 3.42-37.90, all P<0.05). Conclusions:Acceptance and commitment therapy can improve the self-compassion and psychological flexibility, reduce anxiety and depression of the primary caregivers of patients with primary liver cancer.

ABSTRACT

Background Occupational stress has been shown to be an important factor affecting the mental health of workers. The role of affective commitment to the organization and overcommitment to work cannot be ignored. However, there is a lack of research on this topic in China. Objective To explore a potential mediating effect of affective commitment on how occupational stress affects the mental health of medical staff and a potential moderating effect of overcommitment on the mediating effect of affective commitment. Methods A total of 1372 health care workers in a tertiary Grade A hospital in Lanzhou City were selected as study subjects for a cross-sectional survey. The occupational stress, emotional commitment, and psychological distress of the subjects were evaluated by the Effort-Reward Imbalance Scale, Affective Commitment Scale, and Kessler 10 Scale. SPSS 26.0 was used for correlation analysis, mediation analysis, and moderated mediation analysis. Common method bias wasevaluated by Harman one-factor test. Results A total of 1372 questionnaires were distributed, of which 1277 valid questionnaires were returned, with a valid recovery rate of 93.08%. The mean occupational stress score was 1.14±0.23, the mean overcommitment score was 20.26±3.21, the mean affective commitment score was 20.25±3.34, and the mean psychological distress score was 26.26±7.90. The Spearman correlation analysis results showed that occupational stress among medical staff was positively correlated with overcommitment and psychological distress (r=0.153, 0.410, P<0.01) and negatively correlated with affective commitment (r=−0.341, P<0.01); overcommitment was negatively related to affective commitment and positively related to psychological distress (r=−0.107, 0.312, P<0.01); affective commitment was negatively related to psychological distress (r=−0.464, P<0.01). The positive effect of occupational stress on psychological distress of medical staff was significant (b=0.41, t=15.42, P<0.001); affective commitment presented a partial mediating effect on the relationship between occupational stress and psychological distress (effect value=0.13), accounting for 31.71% of the total effect; overcommitment moderated the process of occupational stress-affective commitment-psychological distress (P<0.01). Conclusion Affective commitment of medical staff has a partial mediating effect on the relationship between occupational stress and psychological distress, and overcommitment plays a significant role in moderating the process of occupational stress-affective commitment-psychological distress.

ABSTRACT

INTRODUCCIÓN: El Compromiso Organizacional (CO), se considera un vínculo entre la organización y los trabajadores, e influye en la satisfacción, el desempeño, la productividad y la inversión profesional. OBJETIVO: Determinar el compromiso organizacional en profesionales y técnicos en enfermería adscritos a un hospital del sur de chile, años 2022-2023. METODOLOGÍA: Estudio de corte transversal, en una muestra de 305 funcionarios. La recolección de la información se realizó con el instrumento compromiso organizacional de Meyer y Allen (1991), puntuación según escala Likert de 5 puntos, donde 1 corresponde a "totalmente en desacuerdo" y 5 es "totalmente de acuerdo"; análisis con estadística descriptiva e inferencial. Estudio autorizado por el Comité de ética científica. RESULTADOS: El instrumento logró consistencia interna adecuada, mediante alfa de Cronbach. El CO observado fue "neutral o indiferente" con tendencia a "de acuerdo", según escala Likert. El Compromiso Afectivo (CA) fue superior respecto a los demás dominios. El Compromiso Normativo (CN) del nivel técnico mostró ser fuerte, así como los funcionarios divorciados y viudos. El Compromiso de Continuidad (CC) resultó ser el dominio con promedio más bajo. A mayor edad, aumentó el CA. CONCLUSIONES: los funcionarios mostraron un nivel medio de compromiso organizacional con tendencia a alto, por lo que se debe fortalecer el lazo afectivo del binomio funcionario/organización, de manera que juegue un papel positivo en la calidad de la atención otorgada hacia sus usuarios.


INTRODUCTION: Organizational Commitment (OC) is considered a crucial link between an organization and its employees, significantly impacting satisfaction, performance, productivity, and professional investment. OBJECTIVE: To assess organizational commitment among nursing professionals and technicians in a hospital in southern Chile between 2022 and 2023. METHODOLOGY: This was a cross-sectional study involving a sample of 305 staff members. Data collection utilized Meyer and Allen's (1991) Organizational Commitment tool, employing a 5-point Likert scale where 1 signifies "completely disagree" and 5 indicates "completely agree." Analysis encompassed descriptive and inferential statistics. RESULTS: The instrument demonstrated sound internal consistency, supported by Cronbach's alpha. Observed OC tended towards a "neutral or indifferent" stance with a leaning towards "agreement" on the Likert scale. Affective Commitment (AC) exhibited higher levels compared to other domains. Normative Commitment (NC) among technical-level employees were robust, and particularly notable among divorced and widowed staff. Continuance Commitment (CC) emerged as the domain with the lowest average. AC showed an increase with age. CONCLUSIONS: Staff exhibited a moderate level of organizational commitment with a propensity towards a higher level, suggesting the need to reinforce the emotional bond between employees and the organization to positively influence the quality of care provided.

ABSTRACT

Objective Chronic pain is an important health problem and affects both quality of life and mental health. This study assessed psychological inflexibility, pain intensity, quality of life, anxiety and depression symptoms, self-efficacy, and social support among patients with chronic pain. Method A quasiexperimental design was used to assess six adult participants pre- and post-group intervention (eight sessions) based on acceptance and commitment therapy. Results After the intervention, the patients experienced a reduction in psychological inflexibility, pain, and depression and anxiety symptoms and improvements in quality of life and self-efficacy. The quality of life and depression symptoms and the domains of quality of life and psychological inflexibility were negatively correlated. Conclusion Acceptance and commitment therapy is a promising treatment for the interdisciplinary treatment of the Pain Clinic.


Objetivo Dor crônica é um problema de saúde prevalente que acarreta prejuízos para qualidade de vida e saúde mental. Este estudo avaliou inflexibilidade psicológica, intensidade da dor, qualidade de vida, sintomas de ansiedade e de depressão, autoeficácia e suporte social em pacientes com dor crônica. Método Utilizou delineamento quase-experimental e os seis participantes adultos foram avaliados por meio de instrumentos no pré e pós-intervenção em grupo (oito sessões) com Terapia de Aceitação e Compromisso. Resultados Após a intervenção houve redução da inflexibilidade psicológica e da dor, melhora da qualidade de vida, redução dos sintomas de ansiedade e de depressão e aumento da autoeficácia. Houve correlação inversa entre qualidade de vida e sintomas de depressão e entre domínios da qualidade de vida e inflexibilidade psicológica. Conclusão Conclui-se que a Terapia de Aceitação e Compromisso é um tratamento promissor a ser integrado no atendimento interdisciplinar da Clínica de dor.

SUMMARY OBJECTIVE: The purpose of this study was to investigate the relationship between psychological inflexibility, depression, anxiety, and quality of life in patients with chronic tinnitus without hearing loss. METHODS: The study was carried out involving 85 patients with chronic tinnitus without hearing loss and a control group of 80 individuals. All participants completed the Acceptance and Action Questionnaire-II, the State-Trait Anxiety Inventory-Trait, the Beck Depression Inventory, and Short Form-36. RESULTS: The patient group exhibited higher Acceptance and Action Questionnaire-II (t=5.418, p<0.001), State-Trait Anxiety Inventory-Trait (t=6.592, p<0.001), and Beck Depression Inventory (t=4.193, p<0.001) scores and lower physical component summary (t=4.648, p<0.001) and mental component summary (t=-5.492, p<0.001) scores than the control group. Psychological inflexibility predicted depression, anxiety, and impairment of quality of life. The effect of psychological inflexibility on physical component summary was mediated by depression (β=−0.15, [95%CI −0.299 to −0.017]), while its effect on mental component summary was mediated by anxiety and serial anxiety and depression (β=−0.17 [95%CI −0.344 to −0.055] and β=−0.06 [95%CI −0.116 to −0.100], respectively). CONCLUSION: Psychological inflexibility plays an important role in patients with chronic tinnitus without hearing loss. It is associated with increased levels of anxiety and depression and decreased quality of life.

RESUMO O presente relato aborda a experiência instituída no Instituto Tecnológico de Aeronáutica denominada Reflita, cuja proposta foi a promoção de encontros para a discussão de temas pertinentes à Graduação, a fim de oportunizar a reflexão coletiva sobre as normas acadêmicas e sua aplicação no cotidiano escolar. Utilizou-se, como ponto de partida para os encontros, os documentos legais vigentes e a experiência dos participantes. A estratégia foi implementada e referendada por pesquisa de opinião, que indicou a viabilidade da continuidade da proposta. Alguns desdobramentos foram percebidos, como por exemplo, a proposição do projeto novo aconselhamento e a implantação de um protocolo para atendimento ao aluno em crise, adaptado à realidade do ITA. Conclui-se assim, que a estratégia foi válida, para além da abordagem prescritiva e normativa, apontando a importância de um processo de diálogo com a comunidade acadêmica, intencionalmente aproximando os aspectos legais do cotidiano escolar.

Algunos desdoblamientos fueron percibidos, como, por ejemplo, la proposición del proyecto nuevo asesoramiento y la implantación de un protocolo para atención al alumno en crisis, adaptado a la realidad del ITA. Se concluye así, que la estrategia fue válida, para más allá del abordaje prescriptiva y normativa, apuntando la importancia de un proceso de diálogo con la comunidad académica, intencionalmente acercando los aspectos legales del cotidiano escolar.


ABSTRACT This report addresses the experience instituted at the Instituto Tecnológico de Aeronáutica called Reflita, whose proposal was to promote meetings to discuss topics relevant to graduation, in order to provide opportunities for collective reflection on academic norms and their application in everyday school life. As a starting point for the meetings, current legal documents and the experience of the participants were used. The strategy was implemented and endorsed by an opinion poll, which indicated the feasibility of continuing the proposal. Some developments were perceived, such as, for example, the proposition of the new counseling project and the implementation of a protocol for assisting students in crisis, adapted to the reality of ITA. It is therefore concluded that the strategy was valid, beyond the prescriptive and normative approach, pointing to the importance of a dialogue process with the academic community, intentionally bringing the legal aspects closer to the school routine.

Resumo O artigo propõe uma leitura crítica do contexto histórico urbanístico e social das comunidades de Manguinhos considerando a fundamentação teórico-metodológica do campo da saúde urbana, apoiada no paradigma da determinação social da saúde e do direito à cidade. O recorte de análise para a identificação dos processos críticos de determinação social da saúde considerou a relevância da superação das necessidades habitacionais como condição indispensável para o direito à cidade e à saúde. A constatação da permanência dos processos mencionados, mesmo após os vultosos investimentos em urbanização realizados em Manguinhos, aponta para a necessidade de revisão das políticas públicas relacionadas e a inserção efetiva e emancipatória da população nessas políticas.


Abstract This article proposes a critical reading of the urbanistic and social historical context of the communities of Manguinhos, considering the field of urban health as a theoretical and methodological foundation based on the paradigm of the social commitment to health and the right to the city. Our demarcation of this analysis to identify the critical processes in the social commitment to health considered the relevance of overcoming housing needs as an indispensable condition for the right to the city and to health. The fact that these processes persist, even after the vast investments made in Manguinhos, indicates the need to review related public policies and their effective implementation to improve conditions for this population.

ABSTRACT

Introdução: Estima-se que a prevalência de sintomas ansiosos em cuidadores familiares de pessoas com demência é igual ou maior do que a prevalência de sintomas depressivos, embora a ansiedade tenha sido pouco explorada na literatura voltada para este público. Estudos têm demonstrado que a Terapia de Aceitação e Compromisso (ACT) pode ser utilizada com cuidadores familiares, com efeitos positivos sobre sintomas ansiosos. Entretanto, a ACT ainda não foi avaliada no formato em grupo e tampouco em países em desenvolvimento para este público. Objetivos: O trabalho foi subdividido em dois estudos. O objetivo do Estudo 1 foi revisar a literatura através de uma meta-análise e avaliar quantitativamente a eficácia da ACT em grupo, presencialmente, nos sintomas de ansiedade e depressão de adultos com 18 anos ou mais. O objetivo do Estudo 2 foi adaptar e avaliar os efeitos da ACT, em grupo, nos sintomas de ansiedade de cuidadores familiares de pessoas com demência. Método: No Estudo 1, quatro bases de dados foram pesquisadas em agosto/2018 e uma busca atualizada foi realizada em novembro/2021. Foram incluídos 48 estudos clínicos randomizados (3292 participantes: ansiedade = 34 ECRs; depressão = 40 ECRs). O Estudo 2, é um estudo exploratório randomizado, realizado em um único centro, não-cego. Cinquenta e sete cuidadores familiares de pessoas com demência foram randomizados para o grupo intervenção (n = 29) ou lista de espera (n = 28). O grupo intervenção participou de oito sessões de ACT em grupo, realizadas por vídeo conferência, semanalmente. Os participantes responderam aos questionários de avaliação de ansiedade, depressão, flexibilidade psicológica, qualidade de vida e sobrecarga, em três momentos diferentes: semanas 0, 9 e 20. Análises de regressão múltipla foram realizadas para comparar os escores dos participantes do grupo intervenção e controle, na semana 9 e 20. Os escores da linha de base foram usados como controle nas análises de intenção de tratamento (ITT) e dos participantes que participaram em pelo menos seis sessões de intervenção. Resultados: No Estudo 1 o tamanho de efeito para sintomas ansiosos foi de médio a grande (g=0,52, p < 0,001; 95% IC=0,30 ­ 0,73), já o tamanho de efeito para sintomas depressivos foi de pequeno a médio (g=0,47, p < 0,001; 95% IC=0,31 ­ 0,64). A amostra do Estudo 2 foi composta em sua maioria mulheres, de baixo a médio nível socioeconômico, com uma idade média de 52 anos e mediana de escolaridade de 15 anos. Participantes do grupo intervenção apresentaram pontuações mais baixas para sintomas de ansiedade e depressão, comparados com participantes do grupo controle. Este resultado foi encontrado nas semanas 9 e 20, tanto na análise de ITT, quanto na análise daqueles que completaram seis sessões ou mais. Apenas os participantes que participaram de pelo menos seis sessões, apresentaram redução da sobrecarga nas semanas 9 e 20, além de apresentarem melhora na qualidade de vida na semana 9. Os resultados não indicaram efeitos da intervenção na flexibilidade psicológica. Conclusão: Este estudo oferece evidências a favor da utilização da ACT em grupo, com familiares de pessoas com demência, auxiliando na redução dos sintomas ansiosos e depressivos.


Background: The prevalence of anxiety symptoms is estimated to be equal to or higher than the prevalence of depressive symptoms in family carers of people with dementia. However, anxiety is currently somehow neglected in the carer literature. Previous studies demonstrated that Acceptance and Commitment Therapy (ACT) is effective for treating anxiety symptoms in family carers of people with dementia, but the vast majority of these studies were conducted in developed countries. Objectives: The thesis was divided into two studies. Study 1 was a meta-analysis which aimed to quantitatively examine the efficacy of group-based ACT, delivered face-to-face, on anxiety and depressive symptoms in adults aged 18 or older. The Study 2 adapted and investigated the effectiveness of an ACT group intervention on the anxiety symptoms in Brazilian family carers of people with dementia. Method: In Study 1, four electronic databases were searched in August, 2018 and an update search was conducted in November, 2021. Forty-eight randomised controlled trials (RCTs) were included in this review (3292 participants: anxiety = 34 RCTs, depression = 40 RCTs). Study 2 was a single-centre, unblinded, double-arm exploratory randomised trial. Fifty-seven family carers of people with dementia presenting with mild to severe anxiety symptoms were randomised to the intervention group (n = 29) or waiting list group (n = 28). The intervention group received eight weekly sessions of group ACT, delivered through videoconference. Participants completed the measure of anxiety and depressive symptoms, caregiver burden, psychological flexibility and quality of life at Weeks 0, 9 and 20. Multiple linear regression analyses were performed to evaluate whether there was a significant difference in scores between the intervention and control groups, at weeks 9 and 20, while controlling for the baseline scores using both intention-to-treat (ITT) and completers data analyses. Results: In Study 1 the overall effect size for anxiety symptoms was medium-to-large (g = 0.52, p < 0.001; 95% CI =0.30­0.73), while the overall effect size was small-to-medium for depressive symptoms (g = 0.47, p < 0.001; 95% CI = 0.31­0.64). In Study 2 Participants were mostly women, from low- to middle-income socioeconomic levels, with a mean age of 52 years and median education of 15 years. Participants in the intervention group had lower scores of anxiety and depression symptoms compared to participants in the control condition at weeks 9 and 20 in both the ITT and completer analyses. Only participants who attended six or more sessions (i.e., completer sample) demonstrated reduced caregiver burden at both follow-up points. The completer sample also showed improvement in quality of life at week 9. The results did not show any effect of the intervention on psychological flexibility. Conclusion: This study provides evidence supporting the use of group ACT, delivered through videoconference, with family carers of people with dementia to help them cope better with anxiety and depressive symptoms.

ABSTRACT

Obsessive Compulsive Disorder (OCD) is ranked the top 10 of the most handicapping illnesses by lost income and decreased quality of life. Hence the need to overcome OCD is imminent. With the present case of OCD, Acceptance and Commitment Therapy (ACT) and Exposure Prevention Response (ERP) were used for treating OCD. Assessment was done using Yale Brown Obsessive Compulsive Scale and Weiss Functional Impairment Rating Scale – Self report. Results show that OCD reduced signi?cantly after the treatment.

Este estudo teve como objetivo explorar a percepção de alunos de graduação em relação à sua participação em uma intervenção baseada na terapia de aceitação e compromisso (acceptance and commitment therapy[ACT]), para redução da inflexibilidade psicológica (IP) e do estresse em estudantes universitários. Parti­ciparam da pesquisa 21 alunos, de diferentes cursos de graduação, com idade média de 23,4 ± 5,61, sendo 71,4% do sexo feminino e 28,6% do sexo masculino. Os dados coletados nas entrevistas foram submetidos à análise de conteúdo e organizados em quatro categorias: experiência no programa, utilidade, viabilidade e sugestões e processos da ACT. Os resultados possibilitaram identificar que o programa foi uma experiên-cia positiva para os participantes. Além disso, constatou-se que a intervenção foi útil e viável, e possibili-tou que os estudantes desenvolvessem as habilidades necessárias para lidar melhor com as adversidades do dia a dia, sem que precisassem se afastar dos seus valores pessoais.


This study aims to explore undergraduate students' perception of their participation in an intervention, based on acceptance and commitment therapy (ACT), to reduce psychological inflexibility (PI) and stress in undergraduate students. Twenty­one students from different undergraduate programs participated in the research, with an average age of 23.4 ± 5.61, of whom 71.4% were female and 28.6% were male. The data collected in the interviews were submitted to content analysis and organized into four categories: experience in the program, utility, feasibility, and ACT suggestions and processes. The results made it possible to identify that the program was a positive experience for the participants, the intervention was useful and feasible and enabled students to develop the requirements to better deal with the adversities of daily life without departing from their personal values.


Este estudio tiene como objetivo explorar la percepción de los estudiantes de pregrado sobre su participa-ción en una intervención, basada en la terapia de aceptación y compromiso (acceptance and commitment therapy [ACT]), para reducir la inflexibilidad psicológica (IP) y el estrés en los estudiantes de pregrado. En la investigación participaron 21 estudiantes de diferentes carreras, con una edad promedio de 23,4 ± 5,61, de los cuales el 71,4% eran mujeres y el 28,6% hombres. Los datos recolectados en las entrevistas fueron sometidos a análisis de contenido y organizados en cuatro categorías: experiencia en el programa, utilidad, factibilidad y sugerencias y procesos de ACT. Los resultados permitieron identificar que el programa fue una experiencia positiva para los participantes, la intervención fue útil y factible y permitió a los estudian-tes desarrollar los requisitos necesarios para enfrentar mejor las adversidades de la vida diaria, sin apar-tarse de sus valores personales.

Objetivo: Verificar se os pacientes prostatectomizados são informados sobre a possibilidade de superação da perda da função erétil pelos benefícios da sexualidade humana após a intervenção cirúrgica. Métodos: A técnica utilizada foi a análise temática que consiste na busca dos núcleos de sentido para se construir uma comunicação. O estudo foi realizado em um hospital de referência no tratamento de câncer e envolveu pacientes prostatectomizados no pós-operatório mediato. Resultados: A perda da função erétil pode ser superada pela utilização da sexualidade humana materializada no amor, carinho e compreensão. Conclusão: A escassez de informações sobre os efeitos da prostatectomia em pacientes cirúrgicos inaugura uma contradição no processo de atenção à saúde aos portadores de câncer de próstata. A contradição, advém do fato de que a resposta a esta questão admite as duas possibilidades, mito e verdade. (AU)


Objective: To verify whether prostatectomy patients are informed about the possibility of overcoming the loss of erectile function due to the benefits of human sexuality after surgical intervention. Methods: The technique used was thematic analysis, which consists of the search for meaning cores to build communication. The study was carried out in a reference hospital for the treatment of cancer and involved patients undergoing prostatectomy in the immediate postoperative period. Results: The loss of erectile function can be overcome by the use of human sexuality materialized in love, affection and understanding. Conclusion: The scarcity of information about the effects of prostatectomy in surgical patients opens a contradiction in the health care process for patients with cancer of prostate. The contradiction comes from the fact that the answer to this question admits the two possibilities, myth and truth. (AU)


Objetivo: Verificar si los pacientes prostatectomizados están informados sobre la posibilidad de superar la pérdida de la función eréctil por los beneficios de la sexualidad humana tras la intervención quirúrgica. Métodos: La técnica utilizada fue el análisis temático, que consiste en la búsqueda de núcleos de significado para construir la comunicación. El estudio se llevó a cabo en un hospital de referencia para el tratamiento del cáncer e involucró a pacientes sometidos a prostatectomía en el postoperatorio inmediato. Resultados: La pérdida de la función eréctil puede superarse mediante el uso de la sexualidad humana materializada en el amor, el afecto y la comprensión. Conclusión: La escasez de información sobre los efectos de la prostatectomía en pacientes quirúrgicos abre una contradicción en el proceso de atención de la salud de los pacientes con cáncer de próstata. La contradicción proviene del hecho de que la respuesta a esta pregunta admite las dos posibilidades, mito y verdad. (AU)

Introdução: a desnutrição representa um problema clínico-nutricional frequente em pacientes hospitalizados, sendo que a detecção precoce do risco nutricional torna-se fundamental, pois possibilita à equipe multidisciplinar o início imediato de uma conduta dietética adequada, minimizando o sinergismo entre a desnutrição e os demais fatores clínicos. Objetivo: determinar o risco nutricional, associando com o estado nutricional, estilo de vida e variáveis clínicas de hospitalização de pacientes internados pelo SUS, comparando tais condições entre os sexos. Metodologia: trata-se de estudo transversal. Para avaliação do risco nutricional dos pacientes hospitalizados foi utilizado o protocolo de triagem nutricional Nutricional Risk Screening (NRS) 2002, além disso foram coletados dados antropométricos, clínicos, sociodemográficos e dietéticos para comparação com o risco nutricional. Resultados: a amostra foi composta por 100 pacientes, sendo 50% adultos e 50% idosos, com média de idade de 56,11±19 anos, sendo a maioria do sexo masculino (51%). O risco nutricional esteve presente em 34% dos pacientes avaliados, sendo 58,8% no sexo feminino e 41,2% no sexo masculino. Não houve diferença das demais variáveis quando comparadas entre os sexos (p>0,05). A maior prevalência de risco observada foi em idosos (27%). Conclusão: o risco nutricional obteve maior prevalência nos pacientes idosos, porém não houve diferença entre os sexos. Sabe-se que a presença de comorbidades e uma inadequada aceitação da dieta por parte dos mesmos pode levar a uma piora significativa do estado nutricional.


Malnutrition is a frequent clinical and nutritional problem in hospitalized patients, and early detection of nutritional risk is essential, as it allows the multidisciplinary team to immediately start an adequate dietary approach, minimizing the synergism between malnutrition and other clinical factors. Objective: to determine the nutritional risk, associating it with the nutritional status, lifestyle and clinical variables of hospitalization of patients hospitalized by the SUS, comparing such conditions between the sexes. Methodology: this are a cross-sectional study. To assess the nutritional risk of hospitalized patients, the Nutritional Risk Screening (NRS) 2002 protocol was used. In addition, anthropometric, clinical, sociodemographic and dietary data were collected for comparison with nutritional risk. Results: the sample consisted of 100 patients, 50% adults and 50% elderly, with a mean age of 56.11±19 years, the majority being male (51%). Nutritional risk was present in 34% of the patients evaluated, being 58.8% female and 41.2% male. There was no difference in the other variables when compared between the sexes (p>0.05). The highest prevalence of risk observed was in the elderly (27%). Conclusion: nutritional risk was more prevalent in elderly patients, but there was no difference between the sexes. It is known that the presence of comorbidities and an inadequate acceptance of the diet by them can lead to a significant worsening of the nutritional status.

ABSTRACT

Introducción: Las enfermedades pulmonares intersticiales difusas constituyen un grupo amplio de afecciones, con manifestaciones clínicas, expresión radiológica y alteraciones funcionales del intersticio pulmonar. Objetivo: Describir el comportamiento clínico y radiológico de la enfermedad intersticial difusa sintomática en pacientes diagnosticados con enfermedades reumáticas. Métodos: Se realizó un estudio observacional, descriptivo transversal, con una población compuesta por 33 pacientes con manifestaciones respiratorias secundarias a enfermedades reumáticas entre enero de 2020 a enero de 2021 en el Hospital Universitario Clínico Quirúrgico Arnaldo Milián Castro. La muestra estuvo conformada por la totalidad de los pacientes. El análisis estadístico se llevó a cabo por medio de estadística descriptiva de cada variable. Resultados: Existió un mayor número de mujeres afectadas (75,8 por ciento) por esclerosis sistémica (31 por ciento), artritis reumatoide (27 por ciento) y lupus eritematoso sistémico (21 por ciento). La afectación respiratoria predominante fue de 81,8 por ciento con compromiso intersticial asociada a un patrón radiológico de neumonitis intersticial usual con 5 casos (21,4 por ciento), todos ligados a artritis reumatoide. Para 25 casos (75,8 por ciento) hubo un retraso diagnóstico menor de 1 año; la enfermedad reumática antecedió a la manifestación respiratoria en 21 casos (63,6 por ciento). Conclusiones: El compromiso intersticial fue la manifestación respiratoria más frecuente; los patrones de neumonitis intersticial usual y la neumonía intersticial no específica predominaron como hallazgos imagenológicos. La secuencia diagnóstica apuntó hacia las enfermedades reumáticas como el debut del proceso(AU)


Introduction: Diffuse interstitial lung diseases constitute a wide group of conditions, with clinical manifestations, radiological expression and functional alterations of the pulmonary interstitium. Objective: To describe the clinical and radiological behavior of symptomatic diffuse interstitial disease in patients diagnosed with collagenosis. Methods: An observational, descriptive, cross-sectional study was carried out. The population consisted of 33 patients with respiratory manifestations secondary to rheumatic diseases. The sample consisted of all the patients. Statistical analysis was carried out by means of descriptive statistics for each variable. Results: There was a greater number of women affected (75.8 percent) by systemic sclerosis (31percent), rheumatoid arthritis (27 percent) and systemic lupus erythematosus (21 percent). The predominant respiratory involvement was 81.8 percent with interstitial involvement associated with a radiological pattern of usual interstitial pneumonitis with 5 cases (21.4 percent), all linked to rheumatoid arthritis. For 25 cases (75.8 percent) there was a diagnostic delay of less than 1-year, rheumatic diseases preceded the respiratory manifestation in 21 cases (63.6 percent). Conclusions: Interstitial involvement was the most frequent respiratory manifestation; the patterns of usual interstitial pneumonitis and nonspecific interstitial pneumonia as predominated imaging findings. The diagnostic sequence pointed towards rheumatic diseases were the debut of the process(AU)

RESUMEN Introducción: La anorexia nerviosa es un trastorno grave que causa tasas elevadas de morbilidad y mortalidad. La aplicación de una intervención no voluntaria solo es legalmente admisible si el paciente no es competente. Sin embargo, la evaluación de su capacidad puede ser extremadamente compleja. Ello conlleva que la decisión final pueda verse influida por las actitudes individuales del facultativo. Objetivo: Crear y validar empíricamente un cuestionario en español que permita medir la actitud hacia la capacidad y el internamiento no voluntario y comparar entre grupos categóricos. Métodos: Formaron la muestra 338 profesionales de salud mental. Los ítems fueron validados por grupos de expertos. Se realizaron un análisis factorial exploratorio y comparaciones grupales. Resultados: Se obtuvo un modelo de 13 ítems formado por 3 factores: prointervención, ausencia de capacidad y cronicidad. Los profesionales tienden a creer en la ausencia de capacidad y la necesidad de la intervención no voluntaria, así como en la idoneidad diferencial en virtud de la cronicidad. El respaldo previo a intervenciones involuntarias se relacionó significativamente con los factores prointervención y ausencia de capacidad y la formación en bioética, con la cronicidad. Conclusiones: El instrumento resultante es válido y fiable. Puede ser útil a profesionales, pacientes y sociedad.


ABSTRACT Introduction: Anorexia nervosa is a serious disorder that causes high rates of morbidity and mortality. Involuntary treatments are only legally admissible if the patient is not competent. However, assessing their capacity can be really complex. This implies that the final decision might be influenced by the individual attitudes of the physician. Objective: To create and empirically validate a questionnaire in Spanish that makes it possible to measure the attitude towards capacity and involuntary commitment and compare between categorical groups. Methods: The sample consisted of 338 mental health professionals. The items were validated by groups of experts. An exploratory factor analysis and group comparisons were carried out. Results: Favourable evidence was obtained of a 13-item model consisting of three factors: pro-intervention, lack of capacity and chronicity. Professionals tend to believe in the lack of capacity and the need for involuntary interventions, as well as differential suitability due to chronicity. Having ever supported involuntary interventions was significantly related to the pro-intervention and lack of capacity factors, and training in bioethics to chronicity. Conclusions: The resulting instrument is valid and reliable. Its use can be useful to professionals, patients and society.
